Adolescents presenting to the emergency department for substance use or mental health concerns often face fragmented transitions to outpatient care. Clear discharge planning and referral coordination are critical to sustained engagement.

Our research examines discharge processes, referral completion, and communication strategies that support families following emergency encounters. The goal is to strengthen continuity and reduce loss to follow-up.
